Accident Summary:

 AT APPROXIMATELY 1500 HOURS ON THE 14TH OF SEPTEMBER, WO1 SCHINDLER, AIRCRAFT COMMANDER, AND WO1 LAWRENCE, PILOT, TOOK OFF FROM NINH HOA IN AIRCRAFT NO. 66-15019 OF THE 134TH ASLT HEL CO, WITH A WING MAN. THEY WERE TO PICK UP A KOREAN OBSERVER APPROXIMATELY THREE MILES TO THE WEST. BECAUSE HE HAD BEEN EXPERIENCING POWER PROBLEMS WITH THE AIRCRAFT, MR. SCHINDLER INSTRUCTED HIS WING MAN TO PICK UP THE OBSERVER AND LEAD THE FLIGHT FROM THERE. MR. SCHINDLER THEN SET UP A COVERING ORBIT AROUND THE PZ. SHORTLY THERE AFTER HIS WING MAN CALLED HIM COMING OUT OF THE PZ. MR. SCHINDLER THEN ROLLED INTO A 30 DEGREE TO 35 DEGREE LEFT BANK TO COVER HIS WINGMAN OUT OF THE PZ. HE WAS AT THREE HUNDRED FEET ALTITUDE AND SIXTY KNOTS. AT THIS TIME, HE EXPERIENCED A LACK OF CONTROL RESPONSE AND STARTED LOOSING ALTITUDE. AS HE PULLED PITCH, RPM STARTED BLEEDING OFF AND HIS RATE OF DESCENT INCREASED TO APPROXIMATELY THAT OF AN AUTOROTATION. WO SCHINDLER MANAGED TO MANEUVER THE AIRCRAFT TO A SUITABLE LANDING AREA. AT APPROSIMATELY FIFTY FEET ALTITUDE AND 4800 ENGINE RPM, THE ROTOR BLADE STRUCK A STAND OF BAMBOO ON THE RIGHT SIDE OF THE AIRCRAFT. THE AIRCRAFT STRUCK THE GROUND ON A DIKE LEFT SKID LOW AND NOSE LOW. RATE OF DESCENT UPON IMPACT WAS BETWEEN 1500 AND 2000 FEET PER MINUTE AND AIR SPEED WAS SOMEWHAT LESS THAN 30 KNOTS. AFTER STRIKING THE GROUND, THE AIRCRAFT SPUN ON ITS NOSE 180 DEGREES TO THE RIGHT. SOMETIME DURING THIS SPIN BOTH BLADES WENT THROUGH THE TAIL BOOM, AND THE TRANSMISSION WAS TORN FROM THE AIRCRAFT. WO SCHINDLER STATED THAT HE HAD FULL AFT CYCLIC AT THE MOMENT OF IMPACT. HEADING UPON IMPACT WAS APPROXIMATELY 160 DEGREES AND THE WIND WAS 210 DEGREES. THERE WAS A SMALL FIRE IN THE ENGINE EXHAUST AREA WHICH WAS EASILY EXTINGUISHED.\\
